This document presents a comprehensive framework for supporting Early Permanence (EP) foster carers and children when an EP placement ends, and the child is reunified with their birth family or another carer. Developed in collaboration with Adopt Southwest, Adoption West, and Aspire Adoption Services, the framework is designed to ensure that transitions are handled with the utmost care and sensitivity to minimise trauma for the child and to support EP carers through this emotionally challenging process. The framework is divided into three stages: preparing for the move, making the move, and supporting relationships after the move. It highlights the importance of flexible, child-centered planning, open communication among all parties, and ongoing support for carers as they navigate the complexities of reunification.
